Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg Tablet combines an ARB (telmisartan) that selectively blocks AT1 receptors to reduce vasoconstriction and aldosterone-mediated sodium retention with hydrochlorothiazide (HCTZ), a thiazide diuretic that inhibits NCC cotransporter in distal convoluted tubule to promote natriuresis and reduce plasma volume. Fixed-dose combination provides additive BP reduction ( 18-24/10-14 mmHg) superior to monotherapy for stage 1-2 hypertension, recommended as initial therapy per JNC 8 guidelines when BP >20/10 mmHg above target; improves adherence while reducing stroke and HF risk.
