Colistimethate Sodium Injection IP 2 Million IU is a sterile lyophilized powder vial of the polymyxin antibiotic colistimethate sodium (equivalent to 2 MIU, ~160 mg colistimethate or 60 mg colistin base) for IV/IM reconstitution. It disrupts Gram-negative bacterial cell membranes (especially MDR P. aeruginosa, Acinetobacter, Klebsiella), reserved for life-threatening infections like ventilator-associated pneumonia, bloodstream infections, complicated UTIs when other antibiotics fail. Loading dose 9 MIU then maintenance 9 MIU/day in 2-3 divided doses IV (infuse over 30-60 min); adjust for renal impairment (2-3 MIU q12-24h CrCl <50 mL/min); monitor nephrotoxicity, neurotoxicity (paresthesias).
